{"id":329574,"date":"2025-04-14T12:47:13","date_gmt":"2025-04-14T17:47:13","guid":{"rendered":"https:\/\/www.liveaction.org\/news\/?p=329574"},"modified":"2025-04-12T10:55:32","modified_gmt":"2025-04-12T15:55:32","slug":"more-breastfeeding-support-black-moms-babies","status":"publish","type":"post","link":"https:\/\/archive.liveaction.org\/news\/more-breastfeeding-support-black-moms-babies\/","title":{"rendered":"More breastfeeding support could be pivotal for the health of Black moms and babies"},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p><span style=\"font-weight: 400;\">A new JAMA report <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2832389\">discusses the need for interventions<\/a> in breastfeeding \u2014 and it reveals significant disparities among racial groups.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Black mothers and their babies are at <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2832320\"><span style=\"font-weight: 400;\">higher risk of dying<\/span><\/a><span style=\"font-weight: 400;\"> during the year after birth when compared to their Pacific Islander, Hispanic, white, and Asian peers. Even as overall rates have declined across racial groups, Black babies <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8870826\/\"><span style=\"font-weight: 400;\">remain<\/span><\/a> <i><span style=\"font-weight: 400;\">more than twice as likely<\/span><\/i><span style=\"font-weight: 400;\"> (10.8 deaths per 1,000 live births) to die before their first birthday than white (4.6 per 1,000 live births) and Asian (3.6 per 1,000 live births) children<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p>While this is likely the result of multiple factors, the low rate of breastfeeding among Black mothers deserves consideration.<\/p>\n<h3><span style=\"font-weight: 400;\">What research reveals \u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Black babies are both more likely to die in their first year \u2014 and <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2832389\"><span style=\"font-weight: 400;\">less likely to be breastfed<\/span><\/a><span style=\"font-weight: 400;\"> \u2014 compared to their white peers. While rates of initiation (ever trying to breastfeed), length of breastfeeding (how long a baby is breastfed, regardless of amount) and particularly exclusive breastfeeding (baby receives only breastmilk, whether from the breast or pumped), are generally unimpressive across all racial groups, Black women rank <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2832389\"><span style=\"font-weight: 400;\">lower than their peers<\/span><\/a><span style=\"font-weight: 400;\"> of other races across every single metric.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Regardless of race, the top causes of infant death in the first year of life <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8870826\/\"><span style=\"font-weight: 400;\">include<\/span><\/a><span style=\"font-weight: 400;\"> \u201c<\/span><span style=\"font-weight: 400;\">birth defects, preterm birth, low birthweight, complications with maternal pregnancy, sudden infant death syndrome, and injuries.\u201d <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Among many contributing factors, Black babies are <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC5796743\/\"><span style=\"font-weight: 400;\">more likely<\/span><\/a> <span style=\"font-weight: 400;\">to be born in lower-performing hospitals (and, when needed, to receive care in lower-performing <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6503514\/\"><span style=\"font-weight: 400;\">neonatal intensive care units<\/span><\/a> <span style=\"font-weight: 400;\">or NICUs) and to live in areas that experience shortages of primary care doctors. Research suggests that infant mortality rates <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6487545\/\"><span style=\"font-weight: 400;\">are lower<\/span><\/a><span style=\"font-weight: 400;\"> in states with more primary care doctors.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As of <\/span><a href=\"https:\/\/www.cdc.gov\/breastfeeding-data\/?CDC_AAref_Val=https:\/\/www.cdc.gov\/breastfeeding\/data\/nis_data\/rates-any-exclusive-bf-socio-dem-2021.html\"><span style=\"font-weight: 400;\">2021<\/span><\/a><span style=\"font-weight: 400;\">, only 51.4% of all American Black babies received <\/span><i><span style=\"font-weight: 400;\">any<\/span><\/i><span style=\"font-weight: 400;\"> amount of breastmilk at six months of age, and <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7891211\/\"><span style=\"font-weight: 400;\">another 2021 study<\/span><\/a><span style=\"font-weight: 400;\"> found that just 40% of U.S.-born Black mothers breastfed at all. The same 2021 study reported that by the time a child was six months of age, just <\/span><i><span style=\"font-weight: 400;\">13.3%<\/span><\/i><span style=\"font-weight: 400;\"> of U.S.-born Black mothers and <\/span><i><span style=\"font-weight: 400;\">20.4% <\/span><\/i><span style=\"font-weight: 400;\">of foreign-born Black mothers were exclusively breastfeeding.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This matters when it come to their babies\u2019 short-term and long-term health, because breastfeeding is associated with lower rates of \u201c[a]<\/span><span style=\"font-weight: 400;\">sthma, severe lower respiratory disease, obesity, Type 1 diabetes, acute otitis media (ear infections), sudden infant death syndrome (SIDS), gastrointestinal infections, which can cause diarrhea and vomiting, [and] <\/span><a href=\"https:\/\/medlineplus.gov\/ency\/article\/001148.htm\"><span style=\"font-weight: 400;\">necrotizing enterocolitis (NEC)<\/span><\/a><span style=\"font-weight: 400;\"> (death of intestinal tissue) for preterm infants,\u201d according to the <\/span><a href=\"https:\/\/www.cdc.gov\/breastfeeding\/php\/about\/index.html\"><span style=\"font-weight: 400;\">Centers for Disease Control and Prevention<\/span><\/a><span style=\"font-weight: 400;\">. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">In fact, breastfeeding for at least two months cuts SIDS <\/span><a href=\"https:\/\/uvaphysicianresource.com\/sids\/\"><span style=\"font-weight: 400;\">risk nearly in half<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Why are Black mothers less likely to breastfeed?\u00a0<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">There are many factors likely in play. According to a <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2832389\">JAMA 2025 report<\/a>, &#8220;There are complex historical, structural, and economic factors that research has shown to contribute to disparities in rates of breastfeeding. Examples include lasting psychological impact and stigma of enslaved Black women being forced to act as wet nurses; the marketing tactics of formula companies; and societal messaging that emphasizes the use of breasts for sexual purposes rather than nutrition.&#8221;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The report also notes<\/span><span style=\"font-weight: 400;\">, \u201c<\/span><span style=\"font-weight: 400;\">Zip codes that have higher proportions of Black residents are less likely to have birthing facilities with supportive breastfeeding practices, such as early initiation of breastfeeding, rooming in, limited use of breastfeeding supplements, limited use of pacifiers, and post-discharge support.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Additionally, Black babies are significantly more likely than white babies to <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7891211\/\"><span style=\"font-weight: 400;\">be born to single mothers<\/span><\/a><span style=\"font-weight: 400;\">, and those mothers <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29098488\/\"><span style=\"font-weight: 400;\">tend to return to work early<\/span><\/a> <span style=\"font-weight: 400;\">for financial reasons, even in cases where job-protected maternity leave is available (though not necessarily paid at regular wage rates). These mothers are also more likely to be employed by companies that make <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/25714345\/\"><span style=\"font-weight: 400;\">pumping breastmilk during the workday<\/span><\/a><span style=\"font-weight: 400;\"> difficult.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">On the <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29098488\/\"><span style=\"font-weight: 400;\">flip side<\/span><\/a><span style=\"font-weight: 400;\">,<\/span><span style=\"font-weight: 400;\"> \u201c<\/span><span style=\"font-weight: 400;\">women who used maternity leave experienced a 50% decline in the odds of having their infants hospitalized, having themselves hospitalized, as well as seeing a mental health provider.\u201d<\/span><span style=\"font-weight: 400;\">\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">How can we change this?<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Many things need to change when it comes to the prenatal, antenatal, and postpartum care Black women and babies receive. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Raising rates of breastfeeding is one relatively inexpensive intervention with a high return on investment. In April of 2025, the <\/span><span style=\"font-weight: 400;\">US Preventive Services Task Force issued a <\/span><a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2832389\"><span style=\"font-weight: 400;\">recommendation<\/span><\/a> <span style=\"font-weight: 400;\">urging early and frequent promotion of breastfeeding during prenatal care visits, including breastfeeding classes and in-office education covering<\/span><span style=\"font-weight: 400;\"> \u201cgeneral breastfeeding knowledge and\u2026 the benefits of breastfeeding, practical breastfeeding skills (e.g., latching), and the management of common breastfeeding complications.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Ensuring immediate skin-to-skin contact and assistance with latching in the \u201cgolden hour\u201d after birth are key for raising initiation rates. After mothers leave the hospital, in-person or virtual consultations can help troubleshoot issues such as shallow latch, pain with breastfeeding, etc. Support can be provided by midwives, physicians, nurses, lactation consultants, and even trained peers. Ongoing education can help mothers navigate pumping when they return to work, ensuring compliance with the <\/span><a href=\"https:\/\/www.dol.gov\/agencies\/whd\/fact-sheets\/73-flsa-break-time-nursing-mothers\"><span style=\"font-weight: 400;\">Fair Labor Standards Act<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Live Action News has previously reported on the importance of increased <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/5-reasons-midwifery-care-matter-pro-lifers\/\"><span style=\"font-weight: 400;\">midwifery care<\/span><\/a><span style=\"font-weight: 400;\"> and <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/los-angeles-reduce-infant-maternal-mortality-doula\/\"><span style=\"font-weight: 400;\">doula utilization<\/span><\/a><span style=\"font-weight: 400;\"> to decrease maternal mortality rates, especially for Black women. Midwives and doulas can also be excellent breastfeeding resources. In a <\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3742682\/#:~:text=Among%20African%2DAmerican%20women%2C%2092.7,health%20benefits%20associated%20with%20breastfeeding.\"><span style=\"font-weight: 400;\">2014 study<\/span><\/a><span style=\"font-weight: 400;\"> of doula-supported Minnesota mothers on Medicaid, 92.7% of Black mothers who had doulas initiated breastfeeding. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">What\u2019s good for mom \u2014 in terms of increased satisfaction with the labor and birth experience, decreased C-section rates, and decreased use of pain medication \u2014\u00a0is apparently good for baby as well.\u00a0<\/span><\/p>\n<h3><span style=\"font-weight: 400;\">Black women lead the way<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Some signs suggest that the tide is slowly turning when it comes to increasing breastfeeding rates among Black mothers, and Black women are making it happen.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The Black Breastfeeding Mothers Organization is a preeminent <\/span><a href=\"https:\/\/www.youtube.com\/watch?v=XiTrVcSZSdg\"><span style=\"font-weight: 400;\">advocacy organization<\/span><\/a><span style=\"font-weight: 400;\"> raising awareness and <\/span><a href=\"https:\/\/www.instagram.com\/p\/CwnsI3SJlYz\/?igsh=cjI3aGRvOXJkZnZ2\"><span style=\"font-weight: 400;\">bringing women together<\/span><\/a><span style=\"font-weight: 400;\"> in supportive settings. August is National Breastfeeding Month, and <\/span><a href=\"https:\/\/blackbreastfeedingweek.org\/why-we-need-black-breastfeeding-week\/\"><span style=\"font-weight: 400;\">Black Breastfeeding Week<\/span><\/a> <span style=\"font-weight: 400;\">during that month seeks to <\/span><a href=\"https:\/\/www.essence.com\/lifestyle\/health-wellness\/black-women-breastfeeding-week-black-moms-gallery\/\"><span style=\"font-weight: 400;\">amplify the visibility<\/span><\/a><span style=\"font-weight: 400;\"> and normalize the practice of breastfeeding among Black mothers.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In 2024, Latham Thomas, the founder of <\/span><a href=\"https:\/\/mamaglow.com\/\"><span style=\"font-weight: 400;\">Mama Glow<\/span><\/a><span style=\"font-weight: 400;\">, rolled out a <\/span><a href=\"https:\/\/www.essence.com\/lifestyle\/latham-thomas-certification-program-for-doulas\/\"><span style=\"font-weight: 400;\">First Foods Justice Breastfeeding Certification Program<\/span><\/a><span style=\"font-weight: 400;\"> for doulas, since there are very few <\/span><a href=\"https:\/\/people.com\/parents\/milky-mamas-breastfeeding-cookies-black-women-lactation-consultants-exclusive\/\"><span style=\"font-weight: 400;\">Black lactation consultants<\/span><\/a><span style=\"font-weight: 400;\"> and relatively more Black doulas. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Thomas commented in an interview with <\/span><a href=\"https:\/\/www.essence.com\/lifestyle\/latham-thomas-certification-program-for-doulas\/\"><span style=\"font-weight: 400;\">ESSENCE<\/span><\/a><span style=\"font-weight: 400;\">, \u201cAs a Black mother who breastfed my son for 3 years, I\u2019m deeply proud of the work we did to develop this program for the community. I was blessed to have a mother who was a powerful teaching example of Black breastfeeding, who helped me in the first week postpartum in learning how to feed my newborn. My son and I developed a rhythm and overtime, breastfeeding would become second nature to me.
