Here are the facts."},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p><span style=\"font-weight: 400;\">Many pro-life women seeking to prevent pregnancy may steer clear of oral contraceptives like the Pill, understanding that they can be abortifacient (potentially ending a child&#8217;s life by preventing implantation of the newly-created embryo). These women may pursue intrauterine device (IUD) placement instead, having heard that IUDs are contraceptive only. But is that true? <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Let\u2019s look at how IUDs are supposed to prevent conception<\/span><span style=\"font-weight: 400;\">\u2014<\/span><span style=\"font-weight: 400;\"> also known as their mechanism(s) of action<\/span><span style=\"font-weight: 400;\">\u2014<\/span><span style=\"font-weight: 400;\"> and then at the evidence that they can, or can\u2019t, function as abortifacients.\u00a0<\/span><\/p>\n<p><strong>How do IUDs act as contraception?<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">IUDs along with implants are considered <\/span><a href=\"https:\/\/my.clevelandclinic.org\/health\/treatments\/23269-larc\"><span style=\"font-weight: 400;\">long-acting reversible contraceptives (LARC)<\/span><\/a><span style=\"font-weight: 400;\">, and their use is on the rise, despite an <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/us-contraceptives-rejected-fda-developing-nations\/\"><span style=\"font-weight: 400;\">unsavory and not-so-distant past for IUDs<\/span><\/a><span style=\"font-weight: 400;\"> in particular. IUDs are markedly different from oral contraceptives like the Pill because ovulation prevention is<\/span><i><span style=\"font-weight: 400;\"> not <\/span><\/i><span style=\"font-weight: 400;\">their main mechanism of action, i.e. the main way they work. There are 2 types of IUDs, categorized by the main active ingredient.\u00a0<\/span><\/p>\n<p><em><span style=\"font-weight: 400;\">Paragard Copper-IUD<\/span><\/em><\/p>\n<p><span style=\"font-weight: 400;\">Copper-IUDs are often called more \u201cnatural\u201d because they continuously release copper into the uterus rather than hormones. Paragard is the only brand of copper-IUD sold in the U.S., and according to its <\/span><a href=\"https:\/\/www.paragard.com\/wp-content\/uploads\/2018\/10\/PARAGARD-PI.pdf\"><span style=\"font-weight: 400;\">full prescribing information<\/span><\/a><span style=\"font-weight: 400;\">, \u201c<\/span><span style=\"font-weight: 400;\">Paragard works by preventing sperm from reaching the egg, preventing sperm from fertilizing the egg, or possibly preventing attachment (implantation) in the uterus. Paragard does not stop your ovaries from making an egg (ovulating) each month.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Paragard is unlike the Pill, then, because it <\/span><i><span style=\"font-weight: 400;\">does not prevent ovulation<\/span><\/i><span style=\"font-weight: 400;\">, meaning that a mature egg could logically be available each cycle for potential fertilization by any sperm that the copper doesn\u2019t kill or otherwise incapacitate. If fertilization occurs, the newly formed embryo would be unable to implant in the uterine lining or endometrium due to the hostile, inflammatory environment maintained by the continuous release of copper.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">And indeed, a 2008 European Society of Human Reproduction and Embryology (ESHRE) workshop <\/span><a href=\"https:\/\/academic.oup.com\/humupd\/article\/14\/3\/197\/683294?login=false\"><span style=\"font-weight: 400;\">report<\/span><\/a><span style=\"font-weight: 400;\"> notes, \u201c<\/span><span style=\"font-weight: 400;\">It is very likely that the high degree of efficacy is related in part to the embryotoxic effect of high copper ion concentrations. The associated inflammatory changes also make the endometrium unfavourable for implantation (<\/span><span style=\"font-weight: 400;\">Stanford and Mikolajczyk, 2002<\/span><span style=\"font-weight: 400;\">).\u201d\u00a0<\/span><\/p>\n<p><em><span style=\"font-weight: 400;\">Mirena and other LNG-IUDs<\/span><\/em><\/p>\n<p><span style=\"font-weight: 400;\">The other type of IUD is hormonal and contains the progestin Levonorgestrel (LNG) as its main active ingredient. Mirena, Kyleena, and Skyla\/Jaydess are all LNG-IUDs. As with copper-IUDs, LNG-IUDs<\/span><i><span style=\"font-weight: 400;\"> don\u2019t <\/span><\/i><span style=\"font-weight: 400;\">primarily work by stopping ovulation. According to Mirena manufacturer Bayer\u2019s <\/span><a href=\"https:\/\/labeling.bayerhealthcare.com\/html\/products\/pi\/Mirena_PI.pdf\"><span style=\"font-weight: 400;\">full prescribing information<\/span><\/a><span style=\"font-weight: 400;\"> \u201cBecause the contraceptive effect of Mirena is mainly due to its local effects within the uterus, ovulatory cycles with follicular rupture usually occur in women of fertile age using Mirena\u2026 In a 1-year study, approximately 45% of menstrual cycles were ovulatory, and in another study after 4 years, 75% of cycles were ovulatory.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">How do Mirena and other IUDs work, then? The manufacturer doesn\u2019t exactly know! <\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThe local mechanism by which continuously released LNG contributes to the contraceptive effectiveness of Mirena has not been conclusively demonstrated.\u201d Potential mechanisms of action include \u201cthickening of cervical mucus preventing passage of sperm into the uterus, inhibition of sperm capacitation or survival, and alteration of the endometrium.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Both copper-IUDs and LNG-IUDs are considered highly effective at preventing pregnanc<\/span><span style=\"font-weight: 400;\">y.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The FDA <\/span><a href=\"https:\/\/www.fda.gov\/consumers\/free-publications-women\/birth-control\"><span style=\"font-weight: 400;\">says<\/span><\/a><span style=\"font-weight: 400;\"> of both copper-IUDs and LNG-IUDs, \u201c<\/span><span style=\"font-weight: 400;\">Out of 100 women who use this method, less than 1 may get pregnant,\u201d though a <\/span><a href=\"https:\/\/link.springer.com\/article\/10.1007\/s11606-022-07433-4\"><span style=\"font-weight: 400;\">2022 study<\/span><\/a><span style=\"font-weight: 400;\"> published in the <\/span><i><span style=\"font-weight: 400;\">Journal of Internal Medicine<\/span><\/i><span style=\"font-weight: 400;\"> found that in the first year after insertion, 2.4% of women with a LNG-IUD and 2.99% of women with a copper-IUD experienced an unplanned pregnancy. But if neither type of IUD consistently prevents ovulation, meaning that some fertilizations undoubtedly occur, what can account for their high rate of pregnancy prevention effectiveness?<\/span><\/p>\n<p><strong>How could IUDs be abortifacient?\u00a0<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">From the mechanisms of action for both copper-IUDs and LNG-IUDs, which do <\/span><i><span style=\"font-weight: 400;\">not<\/span><\/i><span style=\"font-weight: 400;\"> stop ovulation and which specifically mention prevention of implantation as a possible mechanism of action,<\/span><i><span style=\"font-weight: 400;\"> the capacity of IUDs to cause early, preimplantation abortions is clear<\/span><\/i><span style=\"font-weight: 400;\">. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">In 1972, thanks to the successful propagandistic efforts of abortion advocacy organizations, the American College of Obstetricians and Gynecologists (ACOG)<\/span><span style=\"font-weight: 400;\">\u2014<\/span><span style=\"font-weight: 400;\"> the professional organization for OB\/GYNs and other women\u2019s healthcare providers<\/span><span style=\"font-weight: 400;\">\u2014<\/span><span style=\"font-weight: 400;\"> arbitrarily <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/acog-abortion-population-control-redefined-beginning-life\/\"><span style=\"font-weight: 400;\">redefined conception<\/span><\/a><span style=\"font-weight: 400;\"> from occurring at fertilization to occurring at implantation of the embryo in the uterus.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">By this erroneous new definition, prevention of implantation, which occurs 6-7 days<\/span><i><span style=\"font-weight: 400;\"> after<\/span><\/i><span style=\"font-weight: 400;\"> fertilization, would not constitute an abortion because conception hasn\u2019t yet occurred. But fertilization<\/span><span style=\"font-weight: 400;\">\u2014not implantation\u2014marks the new existence of a genetically unique, separate individual.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If thickened cervical mucus from the LNG-IUD doesn\u2019t prevent sperm from reaching a newly released egg, or similarly if the copper in a copper-IUD doesn\u2019t incapacitate all the sperm and one does reach a newly released egg, fertilization can occur. But in either case, <\/span><span style=\"font-weight: 400;\">an early abortion would be expected due to<\/span><a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2005\/018680s060lbl.pdf\"><span style=\"font-weight: 400;\"> inability to implant<\/span><\/a><span style=\"font-weight: 400;\"> in the thinned lining of the uterus. <\/span><span style=\"font-weight: 400;\">Even this <\/span><a href=\"https:\/\/www.huffpost.com\/entry\/how-iuds-work-iud-not-abortion_n_565dd057e4b08e945feca2a2\"><span style=\"font-weight: 400;\">Huffington Post article <\/span><\/a><span style=\"font-weight: 400;\">admits that, when used as emergency contraception, copper-IUDs may be abortifacient \u201cin a small number of cases.\u201d\u00a0<\/span><\/p>\n<p><strong>How <i>often <\/i>might IUDs act as abortifacients?<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">A <\/span><a href=\"https:\/\/journals.sagepub.com\/doi\/full\/10.1177\/0024363919854289\"><span style=\"font-weight: 400;\">2020 literature review<\/span><\/a><span style=\"font-weight: 400;\"> sought to answer this very question. The review authors found that most of the studies were old. Even the best studies were only of moderately good quality, and many were low quality, suggesting that this is a necessary area for further research. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">The review authors also observed that research comparing rates of postfertilization but preimplantation pregnancy loss (also called early pregnancy loss or subclinical pregnancy loss, or a very early miscarriage) in women using IUDs versus in normally ovulating women not using any form of contraception (which would be considered spontaneous early miscarriage) is not available. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Nonetheless, previous research does offer some useful information regarding how often IUDs might be abortifacient <\/span><i><span style=\"font-weight: 400;\">and<\/span><\/i><span style=\"font-weight: 400;\"> why truly accurate rates of loss are hard to come by.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While most studies of postfertilization, preimplantation pregnancy loss tested <\/span><span style=\"font-weight: 400;\">\ua7b5-hCG (human chorionic gonadotropin, also known as the pregnancy hormone) levels, the literature review authors noted that these levels only rise <\/span><i><span style=\"font-weight: 400;\">after<\/span><\/i><span style=\"font-weight: 400;\"> implantation. If a study <\/span><i><span style=\"font-weight: 400;\">only<\/span><\/i><span style=\"font-weight: 400;\"> measures these levels, it can\u2019t accurately assess rates of postfertilization but preimplantation pregnancy loss in IUD users. Even still, \u201cOverall, with all women studied in all publications of any quality, 7.3 percent of IUD users had evidence of fertilization and pregnancy failure (see <\/span><a href=\"https:\/\/journals.sagepub.com\/doi\/full\/10.1177\/0024363919854289#table3-0024363919854289\"><span style=\"font-weight: 400;\">Table 3<\/span><\/a><span style=\"font-weight: 400;\">). With only good-quality studies included, 4.5 percent of IUD users had evidence of fertilization and pregnancy failure.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Interestingly, another blood test called Hsp10 or early pregnancy factor \u201cis more likely to detect the presence of early embryos that do not survive to implantation and the point of elaboration of detectable levels of \u03b2-hCG.\u201d\u00a0 Hsp10 is a protein that helps the mother\u2019s immune system accommodate her genetically foreign offspring, and it\u2019s released long before \ua7b5-hCG levels rise. In the one small study of Hsp10 that the authors found from 1992, researchers identified \u201cthe occurrence of fertilization with a probable failure of the fertilized ovum to implant in the endometrium of the uterus in 26 percent of the cycles studied.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The 2008<\/span> <a href=\"https:\/\/academic.oup.com\/humupd\/article\/14\/3\/197\/683294?login=false\"><span style=\"font-weight: 400;\">ESHRE workshop<\/span><\/a><span style=\"font-weight: 400;\"> report similarly concluded, \u201c<\/span><span style=\"font-weight: 400;\">There is sufficient evidence to suggest that IUDs can prevent and disrupt implantation. The extent to which this interference contributes to its contraceptive action is unknown. The data are scanty and the political consequences of resolving this issue interfere with comprehensive research.\u201d\u00a0<\/span><\/p>\n<p><strong>Why isn\u2019t the abortifacient capacity of IUDs more widely discussed?<\/strong><\/p>\n<p><span style=\"font-weight: 400;\">Perhaps the best summary of why IUDs\u2019 ability to act as abortifacients isn\u2019t more widely discussed comes from the 2008 ESHRE workshop report, which tellingly noted<\/span><span style=\"font-weight: 400;\">:<\/span><\/p>\n<p style=\"padding-left: 30px;\"><em>\u2026 [I]f it was conclusively shown that [IUDs can] prevent the embryo from implanting, then [they] would be considered by the Roman Catholic church as causing an early abortion.
