{"id":283832,"date":"2023-01-25T10:38:38","date_gmt":"2023-01-25T16:38:38","guid":{"rendered":"https:\/\/www.liveaction.org\/news\/?p=283832"},"modified":"2023-01-24T16:31:23","modified_gmt":"2023-01-24T22:31:23","slug":"boston-doula-program-maternal-mortality","status":"publish","type":"post","link":"https:\/\/archive.liveaction.org\/news\/boston-doula-program-maternal-mortality\/","title":{"rendered":"Boston doula program aims to decrease maternal mortality rate"},"content":{"rendered":"<div style=\"margin-top: 0px; margin-bottom: 0px;\" class=\"sharethis-inline-share-buttons\" ><\/div><p><span style=\"font-weight: 400;\">A Boston-area healthcare system <\/span><a href=\"https:\/\/www.bostonglobe.com\/2022\/12\/30\/metro\/mass-general-brighams-doula-program-aims-close-racial-disparities-childbirth\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">reported<\/span><\/a><span style=\"font-weight: 400;\"> a successful first year of a pilot program aiming to <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/community-based-doula-program-aims-to-reduce-maternal-mortality-rate-amongst-african-americans\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">reduce maternal mortality<\/span><\/a><span style=\"font-weight: 400;\"> by providing free doula services to women most likely to need, but often unable to afford them. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">A Boston Globe article spotlighted <\/span><span style=\"font-weight: 400;\">Massachusetts General Brigham\u2019s (MGB) <\/span><a href=\"https:\/\/www.massgeneral.org\/obgyn\/news\/striving-equitable-birth-outcomes-doula-care\"><span style=\"font-weight: 400;\">Birth Partners Doula<\/span><\/a><span style=\"font-weight: 400;\"> project<\/span><span style=\"font-weight: 400;\">, which formed in October of 2021 as one initiative among many in BMG\u2019s United Against Racism program. In its first year, the program served 58 Black or Indigenous first-time mothers who <\/span><a href=\"https:\/\/www.birthinggently.com\/_files\/ugd\/91f100_4f850f1ecfa542de92e1c92d9ac2b9ce.pdf\"><span style=\"font-weight: 400;\">were<\/span><\/a><span style=\"font-weight: 400;\"> low-income and homeless, incarcerated, lacking support, or otherwise at-risk. Participants were selected from a lottery that they entered during their third trimester of pregnancy. Each mother received doula accompaniment at two prenatal visits, throughout the labor and birth process, and for two postpartum home visits.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">While other doula programs in the country \u2014\u00a0<\/span><a href=\"https:\/\/www.mass.gov\/news\/hpc-awards-400000-to-combat-racial-inequities-in-maternal-health#:~:text=Award%20Amount%3A%20%24196%2C924%20Boston%20Medical%20Center%20proposes%20to,one%20and%20two%20additional%20doulas%20in%20year%20two.\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">including another Boston area hospital<\/span><\/a>\u00a0\u2014<span style=\"font-weight: 400;\">\u00a0have relied on private donations, grant money, or,<\/span> <a href=\"https:\/\/archive.liveaction.org\/news\/doula-services-reimbursable-through-insurance\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">in rare cases<\/span><\/a><span style=\"font-weight: 400;\">, <\/span><a href=\"https:\/\/healthlaw.org\/current-state-of-doula-medicaid-implementation-efforts-in-november-2022\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Medicaid funding<\/span><\/a><span style=\"font-weight: 400;\">, to operate, the MGB program is the first nationwide to be funded by a healthcare system itself. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">The Birth Partners Doula program funds four hospitals within the MGB system, which then contract with local doula agencies, with an emphasis on matching clients with doulas who have similar \u201ccultural and linguistic backgrounds.\u201d The out-of-pocket cost of doula services, which are typically not covered by insurance, would otherwise be $2,000.\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.liveaction.org\/walgreens-cvs-petition\/\"><img decoding=\"async\" loading=\"lazy\" class=\"aligncenter size-large wp-image-283198\" src=\"https:\/\/archive.liveaction.org\/news\/wp-content\/uploads\/2023\/01\/Keep-Abortion-Pill-Out-Of-Walgreens-CVSLAN-Horizontal--700x180.jpg\" alt=\"Walgreens CVS banner\" width=\"700\" height=\"180\" srcset=\"https:\/\/archive.liveaction.org\/news\/wp-content\/uploads\/2023\/01\/Keep-Abortion-Pill-Out-Of-Walgreens-CVSLAN-Horizontal--700x180.jpg 700w, https:\/\/archive.liveaction.org\/news\/wp-content\/uploads\/2023\/01\/Keep-Abortion-Pill-Out-Of-Walgreens-CVSLAN-Horizontal--300x77.jpg 300w, https:\/\/archive.liveaction.org\/news\/wp-content\/uploads\/2023\/01\/Keep-Abortion-Pill-Out-Of-Walgreens-CVSLAN-Horizontal--768x197.jpg 768w, https:\/\/archive.liveaction.org\/news\/wp-content\/uploads\/2023\/01\/Keep-Abortion-Pill-Out-Of-Walgreens-CVSLAN-Horizontal--500x128.jpg 500w\" sizes=\"(max-width: 700px) 100vw, 700px\" \/><\/a><\/p>\n<h4><strong>Black women are disproportionately impacted by maternal mortality, and rates are worsening over time<\/strong><\/h4>\n<p><span style=\"font-weight: 400;\">While <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/los-angeles-reduce-infant-maternal-mortality-doula\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">maternal mortality rates<\/span><\/a><span style=\"font-weight: 400;\"> are <a href=\"https:\/\/www.commonwealthfund.org\/blog\/2022\/us-maternal-mortality-crisis-continues-worsen-international-comparison#:~:text=New%20international%20data%20show%20the,most%20other%20high%2Dincome%20countries.\" target=\"_blank\" rel=\"noopener\">significantly higher<\/a> amongst U.S. women of every ethnicity compared to those in other developed nations, Black women are the hardest hit by maternal mortality. A <\/span><a href=\"https:\/\/www.cdc.gov\/nchs\/data\/hestat\/maternal-mortality\/2020\/maternal-mortality-rates-2020.htm\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">Centers for Disease Control and Prevention (CDC) report<\/span><\/a><span style=\"font-weight: 400;\"> on the 2020 maternal mortality rate showed an abysmal rate of 55.3 deaths per 100,000 live births amongst Black women, while the rate amongst white women was 19.1 deaths per 100,000 live births. These numbers were <\/span><span style=\"font-weight: 400;\">significantly worse <\/span><span style=\"font-weight: 400;\">than just one year prior, when the rate was 44 deaths per 100,000 live births. The group most at risk of dying during pregnancy, birth, or the postpartum period is Black women over age 40. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">The CDC found that the maternal mortality rate in aggregate (all races) was a staggering 107.9 per 100,000 live births for <\/span><span style=\"font-weight: 400;\">women aged 40 and over<\/span><span style=\"font-weight: 400;\">.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Massachusetts General Hospital is one of the four participating in the MGB Birth Partners Doula program. Senior medical director of health equity, quality, and patient experience Dr. Allison Bryant commented, \u201c<\/span><span style=\"font-weight: 400;\">In particular, our Black and Indigenous birthing people are much more likely to have maternal morbidities, or to be sick in the context of their pregnancies, or to die in the context of their pregnancies or in the year following.\u201d\u00a0<\/span><\/p>\n<h4><strong>Doula services help drive down the maternal mortality rate<\/strong><\/h4>\n<p><span style=\"font-weight: 400;\">In a joint statement, the American College of Obstetricians and Gynecologists (ACOG) and the Society for Maternal-Fetal Medicine<\/span> <a href=\"https:\/\/www.ajog.org\/article\/S0002-9378(14)00055-6\/fulltext\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">recognized<\/span><\/a><span style=\"font-weight: 400;\"> that \u201c<\/span><span style=\"font-weight: 400;\">Published data indicate that one of the most effective tools to improve <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/clinic-improve-birth-outcomes-black-mothers\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">labor and delivery outcomes<\/span><\/a><span style=\"font-weight: 400;\"> is the continuous presence of support personnel, such as a doula.\u201d The statement notes that research shows \u201cthat the presence of continuous one-on-one support during labor and delivery was associated with improved patient satisfaction and a statistically significant reduction in the rate of cesarean delivery [a major abdominal surgery with possible risks of hemorrhage, infection, and even death].\u00a0<\/span><span style=\"font-weight: 400;\">Given that there are no associated measurable harms, this resource is probably underutilized.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pregnant women in Massachusetts have much to gain from doula services. A\u00a0<\/span><a href=\"https:\/\/archives.lib.state.ma.us\/bitstream\/handle\/2452\/859167\/on1322121286.pdf?sequence=1&amp;isAllowed=y\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">2022 report<\/span><\/a><span style=\"font-weight: 400;\"> by the state\u2019s Special Commission on Racial Inequities in Maternal Health estimated that the provision of doula services translates to a 39% decrease in Cesarean sections plus decreased pain medication use and greater rates of breastfeeding, which has<\/span> <a href=\"https:\/\/www.healthline.com\/health\/breastfeeding\/11-benefits-of-breastfeeding\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">numerous benefits<\/span><\/a> <span style=\"font-weight: 400;\">for both mothers and babies.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to a Massachusetts General <\/span><a href=\"https:\/\/www.massgeneral.org\/obgyn\/news\/striving-equitable-birth-outcomes-doula-care\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">press release<\/span><\/a><span style=\"font-weight: 400;\">, doula services are associated with an \u201c<\/span><span style=\"font-weight: 400;\">increase in spontaneous vaginal births and shorter labors, and a decrease in the use of pain medications, regional anesthesia, instrumental vaginal delivery, Cesarean sections, and most importantly, a negative birth experience.<\/span><span style=\"font-weight: 400;\">\u201d\u00a0<\/span><\/p>\n<h4><span style=\"font-weight: 400;\"><strong>How do doulas do it?<\/strong>\u00a0<\/span><\/h4>\n<p><span style=\"font-weight: 400;\">How can <\/span><a href=\"https:\/\/archive.liveaction.org\/news\/new-york-city-doula-services-low-income\/\" target=\"_blank\" rel=\"noopener\"><span style=\"font-weight: 400;\">doulas<\/span><\/a><span style=\"font-weight: 400;\">, who are nonmedical personnel, tackle steep maternal morbidity (short or long-term medical conditions that develop during pregnancy or birth) and mortality rates? They provide emotional, physical, and informational support to pregnant women and their partners or families, teaching them what to expect and what their options are, and then advocating for them, particularly during labor and birth. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mari-Elena Leckel, the\u00a0founder of one of the doula agencies partnering with the Birth Partners Doula program, told the Globe, \u201cFor someone who\u2019s never done this [given birth] before and doesn\u2019t really understand the process, we can break it down and help make decisions so that they can feel like part of the process instead of the process being done to them.\u201d <\/span><\/p>\n<p><span style=\"font-weight: 400;\">Phelicia Cadet, one of the Birth Partners Doula program clients attested, \u201cThe program helps you to understand how the process works and how to prepare yourself.\u201d\u00a0<\/span><\/p>\n<p>&nbsp;<\/p>\n<p><span style=\"font-weight: 400;\">The pro-life movement has long recognized that when women lack support, they may be pressured into abortion. But conversely, when pregnant mothers are supported, their babies thrive. The expansion of doula services nationwide recognizes the vital importance of caring for both mothers <\/span><i><span style=\"font-weight: 400;\">and <\/span><\/i><span style=\"font-weight: 400;\">children, throughout pregnancy, labor, birth, and beyond. Prioritizing both, not focusing on one at the expense of the other, as the abortion lobby does, is a fundamental pillar of a truly just, pro-life society.\u00a0<\/span><\/p>\n<p>Abortion advocates have long falsely insisted that legalizing or expanding abortion would drive down the maternal mortality rate, a claim that Live Action News previously addressed <a href=\"https:\/\/archive.liveaction.org\/news\/maternal-health-expert-abortion-mortality-america\/\" target=\"_blank\" rel=\"noopener\" data-saferedirecturl=\"https:\/\/www.google.com\/url?q=https:\/\/archive.liveaction.org\/news\/maternal-health-expert-abortion-mortality-america\/&amp;source=gmail&amp;ust=1674237293836000&amp;usg=AOvVaw3RcPowoBZHioHeakf-mdRm\">here<\/a>. In reality, we decrease maternal mortality by fixing what&#8217;s actually wrong with the system, not by killing the preborn.
